Bouncing a reinforced and buffed creature seems to be the issue.

It was triggered because you matched green mana, no?
I was talking about the tdw support, not my H2 support.

I was doing the top node in TDW as Hautli G/W and had this freeze just now. This was quite a few turns into the match since I was waiting for creatures, and prior to this turn I had not cast a creature yet.

The game locked when I chose to replace the visionary stack with another creature casting from my hand.

I don't know why it says the permanent support was triggered, you can't use PW abilities during the cast/combat time.
@stikxs imo, this is freeze form replacing reinforced creature.

I get a soft-lock when I play the following cards on a single turn, in order:
    1. Raptor Companion
    2. Squire's Devotion
    3. Raptor Companion
    4. Blink of an Eye (on the reinforced, buffed Raptor Companion)
It seems to be important that the creature is buffed before being reinforced.

While the game is frozen, the Raptor companion is shown as 6/2 lifelink with one reinforcement (as if the +1/+1 from Squire's Devotion was removed but the lifelink remained).

(Teferi vs first encounter of Story Mode. Deck: Raptor Companion, Squire's Devotion, Blink of an Eye, Perilous Voyage, Sphinx's Tutelage, 3x draw spells, 2x gem changer supports. iPhone 6, iOS 12.1.2)

Thanks to ZW2007-, above, for the hint. Hope this helps.

For me it's only happened when things are reinforced, same with replacing creatures, it only bugs with reinforced ones...

So, maybe it's not exactly the bouncing, maybe it something to do with the background code of working out whether reinforcements need to go to the graveyard or something like that that affects both cases?

Perhaps Oktagon can't reproduce it easily because they've managed to fix one of the bugs and can now no longer trigger the other one because it was really related?

(Killing reinforced things works fine, perhaps that uses slightly different code?)

I posted about the reinforce + buff detail last month.

It may not happen 100% of the time when a creature is bounced/replaced while being both reinforced externally buffed (Evra was safe in testing with self buff), but 100% of the crash screens I've seen posted or experienced myself over the last couple months did involve a creature that was both reinforced & externally buffed.

Just had Teferi use his first ability to bounce my Harbinger of Tides stack with 1 reinforcement and lock the game up in R2O. The creature was also buffed by Starfield of Nyx as well.

I submitted a ticket already for this but thought I'd come here to check in too.
I had a very similar experience in RTO as @stikxs mentioned above.
I've only noticed this bounce effect in coalition events that have a special support on the board...but these scenarios might be red herrings, see below.
I hadn't noticed that it only triggers a soft-lock on a reinforced AND buffed critter until I read it here.
The theory is consistent because I'm running pirates in that top node to get their +2/0 synergy.

I have attempted to recreate this issue all over the map for a while now (Training Grounds, Story, Trials Blue, Across Ixalan, Rising Tensions) with no success and I play a lot of flavours of blue...more than I care to admit.

Here's a picture of the board and the log with the opponent's name fuzzed out: https://photos.app.goo.gl/JGuwesoCbPzFtyZJ6 (embed didn't work for me)
I do hope some of this helps.

    The Red Herring
The last time I caused a soft-lock against myself, I was using "Rescue" on a multi-reinforced Startled Awake (before the flip) in the Holiday Showdown event.  I was running Jace II and we all know how insane his buffs can get...again the buffs hadn't played into the equation at the time.  I just thought it was the reinforcement.  I haven't used Rescue since this incident.  But Holiday Showdown wasn't a coalition event and there were no special supports on the board.  They was, however, another special condition in play, super-charge.  Up until then I'd only seen this happen during coalition events.

I have a freeze when startled awake (flipped) try to go to opponent hand that was full and with all cards full of mana. It got killed (0 resistance) but stay in play and game freeze. The first one he cast died normally, but second time the game soft-lock. The only difference was that the second time all cards in his hand were full
