Immortal Hulk perma-death against teams with no ability to do so

I’ve seen it happen now twice in the past two weeks, once in PVP and now against a PVE node. I assumed the PVE instance was due to some sort of support interaction, but now that it’s happened in PVE I’m not so sure. I brought my normal clear team of Okoye (5* RTF)/Omega Red/iHulk into the challenge node here in Fight for Wakanda against 3* Bullseye/4* Elektra/4* Kingpin, and sure enough iHulk perma-died during a random incoming cascade. I’m not really sure what to make of it.
Has there been a quiet nerf to the revive?

    Wait! Breaking news: he just didn’t insta-revive. I made a match after pulling that screenshot and he came back after that resolved.

    That did not happen in the PVP instance mentioned previously.

    Nobody had it in the PVP, and the second time it acted funny was a PVE so no enemy supports at all. That PvP match was Eros/Omnippotence City/and I think the leapfrog was on the third.

    Maybe it was something from omnipotence city. One of the effects is that every time a power of a color is fired for the first time you get a charged tile created then an additional hit of damage. So if a power fired and killed hulk, he came back, but then the additional charged tile hit was enough to kill him he stayed dead.

    That wouldn't explain pve though.

    I don't think it is support related. When ihulk was everywhere when he was released before supports I had experienced this a couple of times atleast. It had always benefitted me so I did not care. It has something to do with the amount of cascades into critical tiles. Anything that is two critical tiles or greater has a chance to trigger it. It is very hard to replicate.

    A good test maybe would be with 2*ip to get it to trigger.

    They do especially for character passives which can trigger part way though a cascade which can then trigger further character passives and so on. It seems that on occasion you can get a problem like IHulk that likely caused him to die mid cascade and the revive happened but likely he was killed a 2nd time as part of the rest of the cascade damage and that didn't trigger the revive because he technically was never 'alive' yet from the earlier revive so the revive passive didn't trigger the 2nd time.

    I thought it monkey red only didn't work when used as a team up
