Does this mean that if, at any point during the game, an opponent's creature was damaged, the ability will summon Demon Berserkers instead?

Or does it mean that if an opposing creature currently on the board was damaged at some point, the ability will summon Demon Berserkers instead?

Or, does it mean that if you manage to damage an opposing creature and get an extra swap, then use your loyalty ability, for that turn it will create Demon Berserkers?

I would expect that it's your second option, since that's effectively the same trigger as the one that gives the Demon Berserker tokens Berserker:
"As long as there is an opposing creature that was dealt damage: This creature gains Berserker."
    I agree it's not well-worded, though.
