Questing Beast doesn't mutate properly [Investigating]

As you can see in the video on the link at the bottom, Questing Beast was mutated with Otrimi, The Ever Playful and my graveyard was packed with creatures. It should have returned me a full mana creature when it attacked, just like Yarok did.

I will do further testing to check if mutations are not working at all with Questing Beast or if it is only failing with Otrimi.
