EotS missing token

In the first wave node (One of them) I had to reset the game to get the rewarded EotS token. This I have never seen before. Playing on Steam.

    Also on Steam: It seems that every wave node gives only two rewards at max without restarting the game. The second fight for a 3ormore wave node without a restart also gives two rewards. If I restart the game after two fights in a 5wave node, I only get one reward, so one reward is still missing.
