    is the code unique to each user, or is it one of their blanket codes, like survey100k?

    Unique to each user, and you cannot even enter the code unless you get the email.

    I still haven't received a survey... does anyone know if they still still being sent out in waves? i.e. has anyone received an email survey this month? 

    100,000 points would be nice!

    unfortunately, the survey is closed for a while. Even u got email today, u cant proceed it.
