BP red passive not activating correctly

BP red is not activating correctly again.
on several occasions, when my BP take dmg above the threshold, he doesn't take partial dmg, doesn't stun himself and doesn't come back to inflict dmg.
Powers that should have activate his passive but didn't
- goon cd tile which deals dmg to a single target and destroy surrounding tile
- Phx red.
