Fetch effects (e.g. Deploy) sometimes not finding creatures?

Sometimes Deploy the Gatewatch will fail to find more than 4 creatures to put into play, and sometimes it doesn't.

I know how the deck system works in this game: You have a 40 card deck, containing 40 copies of each card. So if you draw/fetch 4 copies of a specific card, you will not draw any more copies of that card until you have drawn the rest of those cards, in which case, another 40 cards will be added to your deck.

I thought that when fetching a card, however, if the card wasn't in your deck, it would also cause another 40 cards to be added?

So surely this shouldn't be happening:

Additionally, I have seen Kiora's second ability sometimes fail to fetch a creature (viewtopic.php?f=32&t=57623&p=626616); and I've heard that Saheeli's third ability will fail to put it's full complement of 4 vehicles onto the battletfield sometimes, although that one I've not tested myself (viewtopic.php?f=32&t=57398&p=6250980)

    Dubious Challenge also seems to suffer from this.

    My opponent, Garruk, cast a Dubious Chllenge, and fetched an Undergrowth Champion into play, but I didn't get a creature, despite the fact I was playing with Voldaren Pariah in my deck.

    Upvoting as this is a huge bug that needs looking into.
