Psylocke not creating strike tiles

Fired red ability with 1 fist tile on board and no strike tiles and all I got was the damage. There seems to be something wrong here. (there were plenty of red tiles on board)

The only additional thing I can think of was that I had just fired off her black Psi-Katana ability. Perhaps linked to that.
