I got my yellow Lady Thor from the WOM progression reward this morning and used the cover to make my second Lady Thor 1/1/1. I restarted the game and saw the yellow cover unused and my second Lady Thor was 1/0/1. ****???
This isn't the first time something like this happened. Demiurge, please feed your hamsters!

Create a support ticket, they should be able to fix you up with a replacement cover. There's some hiccups sometimes if your connection to their server wavers (your fault or theirs) while receiving prizes (Iso, Hp, Covers, etc). Annoying, but should be an easy fix. :]0

Hey there Kratos - I didn't create a support ticket because I was lucky enough to check through all the covers I stored up. I didn't have a "check internet connection" or whatever message after I got the cover and after I used it on the 2nd Lady Thor. I'm just doing a community service to let Demiurge and forumites here know that there's a server issue and hope that they can fix it.

It's not a server issue. The game doesn't ping the servers for every user action. Stuff like using a cover, pumping ISO into a character, and using a health pack don't necessarily cause the game to check in with the servers. As a result, if you immediately close the app out, the game might not "remember" that you did it.
I've learned to go into Prologue and cream the Lvl 2 Doombots before I close out the game. Just in case.0 -
Is the cover still missing or did it like just revert to a previous state and the cover just didn't apply? The latter would be really weird, but I guess some latency could cause it to desync, but I would assume there'd be an error message. Very odd. Well, at least nothing was lost!0

if i understand the OP correctly, he used his Yellow 4hor cover to cover up his second 4hor 1/1/1 but when he restarted the game the cover reverted back to the cover stock screen and his second 4hor was 1/0/1. It has happened to me once or twice in the game but i think 808SpicyToro's explanation is sufficient to explain the phenomenon.0

I did not immediately close the app. In fact I made another shield hop, reshielded. Closed the app, opened my heroic 10 pack token pack, and noticed that the cover wasn't used. The points gained during the shield hop was updated. Soooooooooooooo.....0 -
I did something similar lately. Sold all my unneeded covers for ISO and closed the game. Went back in to check something and they were all back again as if nothing had happened.
I simply assumed data hadn't got sent to D3's servers so I tried again, used the ISO and did a couple of PvP rounds and left again.0 -
Just happened to me today. I got the alliance and personal rewards for the WOM event, used the covers on iron fist and gave him some iso. Exited the game and came back a little later. Except then all my rewards were gone, iron fist was uncovered and shivering, and the event screen still shows as rewards pending all day.
I expect if I submit a ticket it will take a week or two to go through, which really sucks because I want to play with my buffed iron fist for his pvp.
Also I still have an open ticket for missing points in the last pve and worry that submitting another ticket will just drop me to the back of the queue.0 -

What I would do is create a new support ticket for the newest issue, then take both of the ticket numbers and message them to HiFi and explain basically what you said here. He can take a look at that older ticker and hopefully get it sorted (or at least give you some insight) and hopefully also poke your new ticket into action at the same time.0
