I Think the very first run flat-out gave out Cap and Iron Man covers as the round 8 reward. But later runs adjusted it so it was just tokens. I focused on only the tokens that have had multiple fivestar characters available though, I suppose I could make a list of all the single-fivestar vaults that have run, but I don't know how useful that information would be.
I will look into it though and see if I can dig anything up.

In the first run, Civil War had two different vaults, one with a 10% chance for Captain America and one with a 10% chance for Iron Man. Tokens cost 30CP. https://forums.d3go.com/discussion/43525/civil-war-details-part-ii
